Empowering Elders: MGM Healthcare Launches ‘Swayam’, Chennai’s First Structured Multidisciplinary Geriatric Programme

Chennai, February 26, 2026: Setting a new benchmark in senior care, MGM Healthcare has launched Swayam, Chennai’s first comprehensive, multidisciplinary geriatric programme built on a structured continuity-of-care model focused on restoring and sustaining quality of life. Swayam integrates outpatient services, dedicated in-patient wards including senior-friendly ICUs, and hospital-to-home care through direct visits, remote monitoring, and structured follow-up for patients at home or in assisted-living facilities.

The programme was inaugurated by Col. K. Prabhakar Hebbar (Retd.), Mrs. Joseph Marita, Branch Head, and Mrs. Krishnaveni V., Dignity Foundation, in the presence of Dr. Senathi Nanda Kishore, Clinical Director & Senior Consultant – Anaesthesiology and Surgical ICU, and Dr. P. Sivaraj, Senior Consultant, Internal Medicine, who heads Swayam.

Distinguishing itself from conventional models, Swayam delivers assessment-driven, coordinated care integrating medical, psychological, functional, nutritional, sensory, dental, and home-based monitoring within one cohesive framework. Services include comprehensive geriatric assessment, chronic disease management, medication rationalisation, preventive health screening, memory and cognitive evaluation, rehabilitation support, metabolic and renal care, infection management, mobility and frailty assessment, and fall-risk evaluation.

Dr. Senathi Nanda Kishore noted that while India’s ageing population is rising rapidly, structured geriatric medicine remains underdeveloped. He emphasised that Swayam aims to shift the narrative from managing decline to optimising healthy ageing through proactive and preventive care.

Dr. P. Sivaraj highlighted that most geriatric care remains fragmented and episodic. In contrast, Swayam follows a true longitudinal model spanning outpatient, inpatient, and home-based services, with an emphasis on sustained monitoring and coordinated multidisciplinary intervention.

Operating on an appointment-based model limited to five consultations per day, Swayam ensures comprehensive, unhurried evaluations, including psychological screening for loneliness and depression. Its Hospital-to-Home component includes quarterly Mobile Health Services, where a doctor, nurse, and physiotherapist conduct structured home visits for continued care and monitoring.
